Scholarship Recipient Information - 2018-2019 School Year
Each scholarship award is divided into two payments, one for Fall semester and one for Spring semester. A few things must be submitted to the office before payments can be issued directly to school (Due to IRS guidelines payment is made out to the school). Please email the following to firstname.lastname@example.org as soon as they are available.
- A screen shot or copy of your full-time registration (at least twelve hours undergraduate, nine hours graduate level) in an accredited program for the Fall installment. The screenshot or copy you send must also show you name.
- A screen shot or copy of your unofficial transcript from Fall 2018 along with a screen shot or copy of full-time registration for Spring 2019 semester for the Spring installment. The screenshot or copy you send must also show you name.
Also, please remember that these funds are contingent upon the following two criteria:
- Remain in good standing with your school (meaning not on academic or social probation)
- Maintain a 3.0 GPA
NOTE: Failure to comply with these requirements by one year from acceptance of your scholarship will result in forfeiture of the scholarship payment.
